Nearly Greatest Worst Aviation Disaster in History

The U.S. Federal Aviation Administration and Air Canada hindered the investigation of last month’s near-catastrophe at San Francisco Airport by dragging their feet in the aftermath.

As a result, key evidence from the cockpit voice recorder was erased and the pilots were never tested for drugs or alcohol. It’s a bureaucratic cover-up that conveniently protects the federal agency and the airline involved.
The fiasco highlights the need for new federal laws or regulations mandating immediate reporting of near-misses and the grounding of aircraft and pilots until after National Transportation Safety Board investigators are called in.

This could have been nearly the worst aviation disaster in history, second only to the two hijacked planes that plowed into the World Trade Center on September 11, 2001.

On July 7, pilots of an Air Canada plane landing minutes before midnight at SFO mistook a taxiway for the runway where they were supposed to land. The latest investigative findings show the plane dipped as low as 59 feet off the ground as the pilots aborted their landing, barely missing four fully-fueled aircraft with an estimated 1,000 passengers that were awaiting takeoff.

The FAA, which was responsible for having only one air controller working traffic in the tower at the time, took more than 24 hours to notify the NTSB. The delay allowed Air Canada to use the plane for three flights in which the cockpit recorder was taped over multiple times.

That recorder held potentially critical information about what the pilots were saying as they headed straight for the taxiway. The cockpit conversation between the pilots might have helped explain their confusion.

As for the pilots, a source familiar with the current NTSB investigation told reporter Matthias Gafni that they spent the night in the Bay Area and flew out the next morning on their normally scheduled flight.

It was business as usual, despicable behavior on the part of Air Canada, which refuses to answer questions during the investigation, including whether the pilots have since been grounded. United Airlines’ outrageous response after a passenger was dragged off a plane pales in comparison to this stonewalling.

Similarly, the FAA refuses to explain why it took more than a day to notify the NTSB. The NTSB, in turn, excuses all this by noting that federal rules did not require that it be notified because there was no collision.

That technical rationalization belies common sense. Air Canada Flight 759 came within a few dozen feet and a few seconds of creating an airport inferno the likes of which this nation has never seen.

Jim Hall, former NTSB chairman, told Gafni that those reporting guidelines should be addressed in the investigation. “This was probably the most significant near-miss we’ve had in this decade,” Hall said. “I think splitting hairs on this issue on an incident of this significance is a disservice to safety.”

He’s right. The investigation into this terrifying episode should have started immediately.

Link of RT between pilots and ATC
http://www.eastbaytimes.com/2017/08/15/edi...-sfo-near-miss/

Dear /K,

Sorry [@amy09] I still do not know how to open a blogspot.

Its been awhile since the last i posted here/

I am already here for more than 2 months, slowly I am picking up their language, culture and lifestyle. Somehow I like it here compare to Malaysia.

I went back to Malaysia for about 2 weeks + during Chinese New Year. I broke up with my girlfriend, it was the worse moment in my life, even now!

Apart of my duty here as a kuli, i have to take care also my boss's business, being able to communicate in Lao language I must say i handle the job quite well now, everything slowly is going on track. I have more times for myself now. Learning Lao language 3 times a week, picking up Golf now (interesting game, regret i didn't pick it up earlier), do sports on weekly basis (football and badminton).

We are involved in Land development project, we buy, clear the land and divide it into smaller lot for resale. Everything goes well now hopefully we can grab as much profit as we can while we can. Now we already proceed to project 5, 5th project now and thank god everything goes well.

[attachmentid=8559812]
Some cash for land clearing fee. Everything is cash basis here, is either local currency/ THB or USD is widely recognized here

[attachmentid=8559814]
Believe it or not, the hut is our sales gallery aka sales office. Buyer will come and view the land and discuss the details at the hut. It is old school but thats the way it works here in Vientiane.

Apart of that I also need to go to KTV a lot. I can only upload 1 photo for reference purpose as i have to take the PNC into consideration. Kinda grand environment but mostly we only have beers instead of whiskey
[attachmentid=8559823]

I have more photos for now.. all photos are in another phone, maybe i will upload again if you all are interested. Just a bit of my experience here in Laos.

So far thats all i guess, need to pack my stuff go home as I am still in the office.

I wish you all happy International Women's!!

Best Regards,
Tuna Seng

Village visit from Vientiane

I came here for land visit today for another village development land project and i had this opportunity to have lunch with locals here. They are Very friendly and fantastic hospitality by them.

The village here is where ppl still practise the traditional way of cooking by using wood for fire.

Their doughthers work as I don't know what's that called but they do something like songket, traditional way. It took them 3 days to finish 1 pcs and it can be sold for 350,000 kip which is equivalent for RM190 each
[attachmentid=8406983]

Right before lunch time all the ladies stop their work and went into the kitchen to prepare our lunch. They have cooking facilities like stove and gas but they still practise the traditional way, I asked them why and they told me it taste better if using wood ( not charcoal).
[attachmentid=8406984]
[attachmentid=8406995]

Once the lunch is ready the ladies served it on our dining table. Very polite, even when cooking they are completely silent I do not know why.. unlike our culture we chit chat laugh and gossip.
[attachmentid=8406986]

Now is where women belongs to the kitchen part. Elderly eat on dining table. Young ladies all squeeze in the small wet kitchen and sit on a stool have their lunch. Very quite even talking sounds like whisper.
[attachmentid=8406998]
Because we started to eat first so we finished our lunch before them. The ladies stop their lunch and come to pick up our dish and clean our dining table

The house owner got one dotter merried to Italian guy ..
[attachmentid=8407015]

From what I see here the local ladies are very obedient, polite and definetely waifu material. They will do everything for you.

I got to continue my work. That's it for today.

Inb4 /K is my blogspot [@amy09] pls forgive me I still don't know how to open blogspot

Kthxbye guys

Best regards,
Tunaseng from Vientiane

My life as an expat in Vientiane

Back then in Malaysia, I had a steady job, plus part time grabcar and some side business, i am struggling but i still manage to live.

My age is catching up, my energy is unable to cope up with the work load, plus i am having a complicated relationship issue, so I decided to leave everything in Malaysia and move over here for a fresh start. I am given an opportunity to work here. I am given a house and some fund to furnish my house. First day reaching i bought queen size mattress, bed sheet, pillow and some cleaning tools to clean the house. Everything seems ok for me on the first day.

It is not as easy as i thought, first of all of course is communication. Most of the locals here they do not speak English. Unlike Malaysia we have mamak stalls 24 hours, fastfood everywhere, here i do not even know what is the local restaurant is selling, plus i have difficulties to communicate with them. Hence, the second day itself i bought a fridge and a stove to settle my daily meals.

I am here for almost a week and guess what, I fell sick on the second day of arrival itself, i feel helpless sometimes when i got nobody to talk to and i am still not recover till today.

Somehow i feel relief to stay here and leave all my stress back in Malaysia, bad things about here is i do not have any friends, there is one but i cannot rely on him everyday... it is torturing and it is not as easy as i thought.

I hope i made the right decision here. I do not feel any regret yet but I am not happy here either. Homesick is real.
