            <title>Why doctor is not making big money</title>

            <description>I HAVE just retired from general practice. Allow me to put the GP charges into proper perspective. I am not even talking about cost of living, inflation, cost of a medical education or the doctor’s worth.&lt;br /&gt;&lt;br /&gt;Generally, GPs would be doing reasonably well if they see 20 to 30 patients a day. Based on a consultation fee of RM30 per patient, with no money being made on drugs and working 26 days a month, the clinic’s income would be between RM15,600 and RM23,400 monthly. If we factor in the wages of two nurses in the clinic working for a minimum pay of RM1,000 per month each, plus their EPF, Socso and EIS, overheads of water, telephone and electricity (no air conditioning in the waiting area) and maintenance costs, rental (depends on location), annual assessment and local licence fees, indemnity insurance (now RM3,200 annually), disposables (syringes, needles, speculum and etc.), waste disposal services and others, the monthly expenditure would be about RM10,000 (if the rental is only RM2,000 per month).&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;So, in a slow month, the doctor’s income would be RM5,600 and RM13,400 in a good one. This does not take into account the cost of replacing equipment and payments for the doctor’s car/house/PTPTN loans (if any).&lt;br /&gt;&lt;br /&gt;Consider that in some places, rental can be as high as RM10,000 and the salary for a nurse could be more than RM2,000 a month. Bear in mind too that the recommended charges are standardised all over Malaysia, be it in Kuala Lumpur or Parit Buntar in Perak.&lt;br /&gt;&lt;br /&gt;Rightly or wrongly, GPs have been charging a consultation fee of between RM10 and RM15 while “making money” from the cost of medicine. The total cost of a visit to a doctor would actually be similar to a consult fee of RM30 plus the cost of medicine, which means GPs are not overcharging their patients.&lt;br /&gt;&lt;br /&gt;I have been charging my patients a consultation fee of RM30 for many years and when they ask, I explain to them the rationale behind the charges. Cost of medicines is an open secret and they can buy these from the pharmacy if they so wish. I would readily write the prescriptions for them.&lt;br /&gt;&lt;br /&gt;The consultation fee is for the history taking, examination, management and advice to the patient. This requires the doctor’s knowledge, expertise and experience. If patients think they only need to pay for the cost of medicines when they go directly to the pharmacy, then it is their choice. But they should remember that when they come to see the doctor at an advanced stage of their disease, they are solely responsible and cannot beg ignorance.&lt;br /&gt;&lt;br /&gt;Even doctors are advised not to treat themselves because we cannot be objective in our self appraisal.             <description>PUTRAJAYA: A senior citizen was jailed 12 years by a Sessions Court here for inserting his finger into an 11-month-old baby girl&amp;#39;s private part last month.&lt;br /&gt;&lt;br /&gt;Judge Mohamad Kamil Nizam also ordered the 68-year-old former lorry driver Hashim Karim to be whipped twice, and instructed the sentence to run from Aug 22, the day Hashim was nabbed.&lt;br /&gt;&lt;br /&gt;ADVERTISEMENT&lt;br /&gt;&lt;br /&gt;Judge Mohamad Kamil meted out the sentence after Hashim admitted to the charge yesterday.&lt;br /&gt;&lt;br /&gt;Hashim was accused of committing the offence at a house in Shah Alam at about 4pm on Aug 20 and was charged under Section 377CA of the Penal Code for sexual connection by object.&lt;br /&gt;&lt;br /&gt;Earlier, in mitigation, Hashim who was clad in olive green shirt and white short pants, said he was unemployed and had to take care of his school-going child.&lt;br /&gt;&lt;br /&gt;Meanwhile, Deputy Public Prosecutor Aimi Syazwani Sarmin urged the court to mete out a deterrent sentence to serve as a lesson to the accused as the crime involved an infant.&lt;br /&gt;&lt;br /&gt;&amp;quot;The victim is like a granddaughter to the accused, and he should have taken a good care of the baby instead for causing injury to her.&lt;br /&gt;&lt;br /&gt;&amp;quot;Although he is 68 years old, he is not spared from being whipped as he is a sexual offender. This is allowed under the law,&amp;quot; she said.&lt;br /&gt;&lt;br /&gt;Aimi also applied to the court to restrain the media from publishing identities and information related to the victim under Section 14 of the Evidence of Child Witness Act 2007.&lt;br /&gt;Judge Mohamad Kamil granted her application.&lt;br /&gt;&lt;br /&gt;Based on the statement of facts, the baby&amp;#39;s parents sent her to babysitter&amp;#39;s house every day, except weekends.&lt;br /&gt;&lt;br /&gt;On Aug 20, they sent their baby to the babysitter&amp;#39;s house as usual. At 4pm, the babysitter asked the accused, her father-in-law, to look after the baby as she needed to renew her road tax and car insurance.&lt;br /&gt;&lt;br /&gt;When the babysitter returned, she found the infant crying continuously in the cradle and saw that there was blood on the baby&amp;#39;s pants.             <description>KUALA LUMPUR: The Attorney General has no hand in the decision to withdraw the corruption charges against Finance Minister Lim Guan Eng and a businesswoman, resulting in the two being acquitted from the charges on Monday (Sept 3).&lt;br /&gt;&lt;br /&gt;In a statement Tuesday (Sept 4), DPP Datuk Mohamad Hanafiah Zakaria said AG Tommy Thomas had recused himself in all deliberations regarding the case.&lt;br /&gt;&lt;br /&gt;ADVERTISEMENT&lt;br /&gt;&lt;br /&gt;&amp;quot;The AG had no hand in the decision to enter nolle prosequi (where Public Prosecutor does not propose further to prosecute the accused) on the charges against Lim and Phang Li Koon,&amp;quot; he said.&lt;br /&gt;Mohamad Hanafiah is the head of the appellate &amp;amp; trial division at the Attorney General&amp;#39;s Chambers.&lt;br /&gt;&lt;br /&gt;He said he decided on the representations from Lim and Phang and the withdrawal was due to the evidence in the case having been substantially weakened.&lt;br /&gt;&lt;br /&gt;&amp;quot;I have perused the evidence that has been investigated by the Malaysian Anti-Corruption Commission (MACC) and the evidence that have adduced and tested under cross examination thus far, I concluded that as a result of the cross-examination of the prosecution witnesses who has testified so far, the evidence supporting the first charge under Section 23 of the MACC Act and under Section 165 of the Penal Code has been substantially weakened.&lt;br /&gt;&lt;br /&gt;&amp;quot;This conclusion was arrived in light of fresh evidence that have arisen during the cross-examination of prosecution witnesses,&amp;quot; he added.&lt;br /&gt;&lt;br /&gt;On Monday (Sept 3), the Penang High Court acquitted both Lim and Phang of the corruption charges over the purchase of the bungalow at below market value.&lt;br /&gt;&lt;br /&gt;The prosecution had dropped the charges and sought an order for both accused to be discharged not amounting to an acquittal.&lt;br /&gt;&lt;br /&gt;The joint trial of Lim, who is former Penang chief minister, and Phang, was in connection to the conversion of land from agriculture to residential status, and the purchase of a plot of land and a bungalow at below market value.&lt;br /&gt;&lt;br /&gt;He was charged with using his position as chief minister to gain gratification for himself and his wife Betty Chew by approving the application for conversion of agricultural land to a public housing zone in Balik Pulau to Magnificent Emblem Sdn Bhd.&lt;br /&gt;&lt;br /&gt;&lt;a href='https://www.thestar.com.my/news/nation/2018/09/04/ag-has-no-hand-in-court-decision-over-guan-engs-acquittal/' target='_blank'&gt;https://www.thestar.com.my/news/nation/2018...engs-acquittal/&lt;/a&gt;</description>

It ought to be emphasised that the decision in respect of initiating prosecution or discontinuing it is a matter within the prerogative and powers of the Public Prosecutor. In this case, I kept confidential my decision until the very last minute and did not consult the investigative agency fearing it might leak and cause unnecessary alarm. In fact, my decision was so confidential that I only informed the AG personally at 9.44am yesterday.&lt;br /&gt;&lt;br /&gt;8. I wish to stress that I have decided the representations without any influence from any quarters. In fact, there was a letter from a legal firm representing the complainant in the above-said case objecting to any idea of withdrawing the charges against Lim Guan Eng and Phang Li Koon. I have responded to the letter that I will decide the case based on available evidence and governing law,  which I did, without fear or favour.&lt;br /&gt;&lt;br /&gt;9.             <description>KUALA LUMPUR: Prime Minister Dr Mahathir Mohamad today vowed that the Pakatan Harapan (PH) government would return the country to its glory days with the Malaysia Incorporated (Malaysia Inc) concept under which the country is seen as one large corporation.&lt;br /&gt;&lt;br /&gt;“(Under this concept) the home country is regarded as one big operation and the government and private sector work together to make profits for this national corporation.&lt;br /&gt;&lt;br /&gt;“From here, we will have the money to support industries to support the people and provide good governance,” he said at the launch of the 9th International Rubber Glove Conference and Exhibition 2018 titled “Transforming Technology, Driving Innovation” at the Kuala Lumpur Convention Centre here.&lt;br /&gt;&lt;br /&gt;The Malaysia Inc concept was first introduced in 1983 as an industrial strategy to forge a symbiotic relationship and partnership between the public and private sectors.&lt;br /&gt;&lt;br /&gt;It is modelled after a Japanese model of economic development to forge mutual public-private partnerships and to target state investment.&lt;br /&gt;&lt;br /&gt;Mahathir said the government encouraged business and believed in the concept of Malaysia Inc.&lt;br /&gt;&lt;br /&gt;“We believe in the government and the private sector working closely together,” he said, adding however that it was not altruistic of the government to help the private sector.&lt;br /&gt;&lt;br /&gt;“If we help you to make more money, it is because 26% of the money you make belongs to the government for it to provide services.”&lt;br /&gt;&lt;br /&gt;He also praised Malaysia for its leading role in the rubber industry, saying the country was now synonymous with rubber.&lt;br /&gt;&lt;br /&gt;“It is not the rubber that is important but the application,” he said, giving the example of surgical gloves and the structure of buildings in earthquake-prone areas.&lt;br /&gt;&lt;br /&gt;He also drew laughter by remarking that rubber is an important component of birth control.&lt;br /&gt;&lt;br /&gt;“Imagine the world without condoms. Courts are only meant for the opposition.”&lt;br /&gt;&lt;br /&gt;He disclosed that many reporters had asked him to comment on the attorney-general’s (AG) decision to withdraw the charge against Lim and the court’s decision to free him.&lt;br /&gt;&lt;br /&gt;“My comment is this: let the people judge,” he said.&lt;br /&gt;&lt;br /&gt;Judge Hadhariah Syed Ismail yesterday decided to discharge and acquit Lim and businesswoman Phang Li Koon of related corruption charges although the prosecution had asked her to consider only a discharge not amounting to acquittal.&lt;br /&gt;&lt;br /&gt;Hadhariah agreed with defence counsel that it would not be right to leave a cloud hanging over their heads.&lt;br /&gt;&lt;br /&gt;Lim was charged on June 30, 2016, with using his public office to obtain gratification for himself and his wife by approving an application by Magnificent Emblem to convert agricultural land for residential purposes during a state exco meeting in July 2014.&lt;br /&gt;&lt;br /&gt;He was also charged with using his position to obtain gratification by purchasing a bungalow from Phang at RM2.8 million, which was said to have a market value of RM4.7 million.&lt;br /&gt;&lt;br /&gt;Phang was charged with abetting him.&lt;br /&gt;&lt;br /&gt;Another Umno personage, Rembau MP Khairy Jamaluddin, criticised the AG’s office for what he called its “selective non-prosecution”.&lt;br /&gt;&lt;br /&gt;“Shame on the Attorney-General’s Chambers (AGC),” he said. “This decision stinks to high heaven and undermines the chambers’ commitment to the rule of law.”&lt;br /&gt;&lt;br /&gt;Pengerang MP Azalina Othman Said, who heads Umno’s legal unit, accused AG Tommy Thomas of giving preferential treatment to PH leaders.&lt;br /&gt;&lt;br /&gt;“The decision to acquit and discharge Guan Eng puts a damper on the credibility of the AG’s office,” she said in a press statement.&lt;br /&gt;&lt;br /&gt;“The AG must explain the matter clearly as this could give rise to a negative perception which can be a threat to the doctrine of separation of powers in the country and result in the people losing trust in the country’s administrative system.”&lt;br /&gt;&lt;br /&gt;She noted that deputy public prosecutor Abdul Rashid Daud, in his opening statement at the start of the corruption proceedings, had said the prosecution would prove Lim had bought a house from Phang at a price known to be below market rate and that he had, at the time the purchase was made, allowed an application by Phang’s company to change the status of agriculture land to one meant for housing.&lt;br /&gt;&lt;br /&gt;“So this begs the question of how the AGC can say there is no case against the two of them and withdraw the charges.             <description>PUTRAJAYA: Prawns, lobsters and shrimps will no longer be subject to the 10% sales and services tax (SST), Finance Minister Lim Guan Eng said today, adding that there had been an oversight by his ministry in the list of taxable seafood.&lt;br /&gt;&lt;br /&gt;Lim said the items were ingredients in popular meals such as Char Kuey Teow and could have a chain effect on food prices.&lt;br /&gt;&lt;br /&gt;“I must confess I like Char Kuey Teow,” Lim told a press conference today.&lt;br /&gt;&lt;br /&gt;He said many of these items were used in curries and dishes sold at Nasi Kandar shops and would now be more expensive.&lt;br /&gt;&lt;br /&gt;“Some have questioned why raw ingredients like prawns, crabs and shells have been hit with the 10% SST.&lt;br /&gt;&lt;br /&gt;“The 10% tax was imposed on them because they were lumped together with luxury items,” Lim added.&lt;br /&gt;&lt;br /&gt;Among the “luxury items” are Rock Lobster, Norway Lobster, cold water shrimps and prawns, oysters and abalone mushrooms.&lt;br /&gt;&lt;br /&gt;Lim said he did not realise taxing regular seafood and other produce would affect the prices of meals at restaurants.&lt;br /&gt;&lt;br /&gt;The SST came into force on Sept 1, more than three months after the government effectively abolished the goods and services tax (GST) by zero-rating it.&lt;br /&gt;&lt;br /&gt;Lim said the 10% tax was being removed now to help traders and consumers who were confused by the SST implementation.&lt;br /&gt;&lt;br /&gt;“Those from the middle and low income groups can now enjoy this without paying the 10% SST,”&lt;br /&gt;&lt;br /&gt;&lt;a href='https://www.freemalaysiatoday.com/category/nation/2018/09/03/regular-seafood-now-sst-free/' target='_blank'&gt;https://www.freemalaysiatoday.com/category/...d-now-sst-free/&lt;/a&gt;</description>
