Making A Complaint agaist a doctor's misconduct

I need help on making a complaint.

The Story:
1. Dad is having kidneys failure
2. Went through govt hospital did dialysis in the past few week in the hospital
3. Dad is now discharged from hospital
4. Doctor advise us to go to a private dialysis center to do his weekly dialysis
5. Went to Private dialysis center. They said need blood test report only can do Dialysis. Doctor did not mention about the blood test report. So request the govt hospital to provide the report.
6. No blood test report given for a week. Dad became week coz no dialysis was done. Called ambulance to send in to er.
7. Spoke to this Indian doctor and he interrogate and said how come you don't know about the blood test? Well i"m not a doctor u should tell us about the blood test and u did not provide it.. how the hell do we know???
8. Doctor went off and no information on blood test report given till now.. dad is now still in er.

Question: How do i make a complaint against this doctor for his misconduct of work?

Sekalipun dihukum penjara lima tahun kes kesalahan meliwat, Ketua Umum PKR Datuk Seri Anwar Ibrahim dijangka muncul di khalayak umum di Mahkamah Tinggi Kuala Lumpur pada Selasa ini, seminggu selepas dijatuhi hukuman itu semalam.

Perkara tersebut disahkan oleh peguamnya J Leela kepada Malaysiakini hari ini.

Anwar yang juga bekas timbalan perdana menteri akan dibawa untuk memberi keterangan dalam saman RM100 jutanya terhadap Menteri Luar Datuk Seri Anifah Aman.

Selain Leela, ketua pembangkang itu turut diwakili peguam Razlan Hadri Zulkifli.

Anwar diarah memberi keterangan lagi susulan dakwaan Anifah dalam pembelaannya bahawa beliau (Anifah) ditawarkan RM100 juta untuk memujuk beberapa anggota parlimen BN dari Sabah untuk melompat menyertai Pakatan Rakyat dalam isu 16 September 2008.
