Violation of contract by Japanese company

I'm working for a Japanese company in Cyberjaya.
I have submitted my claim for AIA insurance 4 months ago and didn't get any reply for it.
HR staff doesn't know the status about it, and just tell me to wait for long time.
AIA staff told me that they didn't receive my claim yet when I contacted to AIA customer service.

Insurance coverage is the guaranteed benefit with the contract.
I'm thinking my company is acting the violation of contract.

Is anyone know where I can report it to?
