First time shipping overseas to Canada, need some advise

Planning to use SF Express as its way cheaper than others (RM100), 2nd option is DHL RM150 without insurance (with insurance add another RM50).

Anyone who has experience with SF, want to ask:

1. Any other form to fill up other than Pro Forma invoice?
2. SF Express international shipping reliable?
3. I did not see any insurance option for SF, is it auto included or not offered?
4. Any other cheap reliable alternatives?
