Refused entry into Malaysia

An old friend of mine together with his wife and their young daughter was scheduled to arrive KLIA2 from Suvarnabhumi Airport on 27/5. The family is based in the Bahamas but decided to go on a holiday in a few countries before reaching Malaysia where the wife was attending a workshop in KL. They had flown from Bahamas to US, US to Netherlands and Netherlands to Thailand with no problems whatsoever. However, when they reached KLIA2, our immigrations officer denied my friend entry into the country because of a small tear in his passport (He asked that I not divulge the country from which the passport was issued but (1) he's from EU and (2) the passport is valid).

The family decided that he will fly back to their last port of entry/exit (i.e. Thailand) with their daughter while the wife carries on with her workshop. Unfortunate for him, since there was now a record that he was denied entry into Malaysia, the Thai immigration officers too are now denying him entry into Thailand! all this because of a small tear in his passport!

I'm not entirely sure what exactly is the criteria for rejecting foreigners entry into Malaysia but in my friend's case: (1) there was clear records on his passport that he had very recently entered other countries with no issue (2) save for a small tear the entire passport was completely legible (3) according to him the immigrations officers had treated him and his family with disdain (he was white but his wife was Bahamian).
