That day my HR contacted me that LHDN sent them an email saying I have to pay tax amount RMxxxx from my selling of my apartment in May 2021. Every month deduct from my salary for 4 months.

hand, arm and shoulder pain

I have pain in my right shoulder and my wrists for 2 months and recently seems getting worse and got needle poking. Went to clinic and get the some medicine ie anti inflammation, vit b, pain. After finished medicine, still not much better. Then wrote me referer letter to orthopedic. orthopedic said I got frozen shoulder. I also told him my both arms and my wrist also pain. Gave me 2 injections and said that it will help 50% and then ask me to do physio and next appointment is one month later.

Everyday, I use keyboard and mouse a lot as I am a software engineer. But I feel more pain when on my wrist when I type and use my mouse. Not sure if I got carpal tunnel and what are other possibilities? What specialist should I go?

Previously I consulted Dr ranjeev from sjmc. After consultations, He recommended me to Dr Cha for fundoplication operation because my gerd is stage 2/3 which I agree. But after 5 months, my gastro still feel discomfort and like got gastric/gerd symptoms. I have done blood test 2 months ago. No h. pylori.

Any good gastrologist to recommend? So that I can get second opinion. I also wonder if the fundoplication is successful or not

Forward to: You who are about to be vaccinated

This article is mainly to help everyone better understand the entire vaccination process, what to prepare beforehand, and what to pay attention to afterwards; most importantly, after reading this article, you will not worry about falling asleep on the eve of vaccination. Feel, or shrink back, because if it happens, it will be a pity.

✔️Preparation before vaccination:

1) Please make sure you understand the possible side effects of vaccination:
(Minor side effects) No matter which vaccine you get, most of the minor side effects that may occur are the same:
-headache
-Chills and fever
-Exhausted
-Muscle ache
-Local swelling and pain at the injection site

(Severe sequelae)
-Pfizer: Seriously sensitive side effects (0.0004%)
-Astrazeneca: Thrombus problem (0.0009%)
***The risk of thrombosis in patients diagnosed with new crown is 16.5%, which is 16 thousand times higher than the risk of vaccination

2) Make work arrangements:
Based on the feedback from most of the people around me, the conclusion is that almost everyone has mild symptoms, and the symptoms of the second dose are more obvious than the first dose. In order not to affect work, it is recommended to arrange a day off with the company after knowing the date of vaccination. If you cannot take a vacation, try to arrange a relaxed itinerary. (Update 10/5/2021: The symptoms of the first dose of Astrazeneca will be more obvious, while the pfizer is in the second dose. You must be mentally prepared; on the other hand, it is not recommended that you ask your colleagues to vaccinate on the same day, because The chance of being absent together the next day will be high)

3) Pregnant women/patients with chronic diseases/persons with a history of severe allergies:
Please consult your doctor whether it is suitable for vaccination, whether to continue taking the medicine or stop.

4) The day before vaccination:
It’s important to eat well and sleep well, and keep a happy mood, because the vaccine will stimulate the immune system to respond, lack of sleep can lead to a poor immune system, and uncomfortable symptoms will be more obvious.

✔️On the day of vaccination:
***Never be late or too early, this will disrupt the process and affect everyone

1) Remember to bring IC and phone (kiv warm clothes + pen + disinfectant):
-I won’t explain why I want to bring it
-The phone is used to scan mysejahtera, because after vaccination, you will receive the certificate of the vaccine, all electronic
-Air conditioners in most hospitals are abnormally cold, and warm clothing can sometimes save lives
-A pen is needed to do the questionnaire + sign the consent form. The venue will provide the pen, but there will be thousands of people using the same pen that day, so bring your own safety

2) Don't go on an empty stomach:
You may be over-excited or over-stressed, and you will be hungry. At the moment of the injection, your blood sugar may suddenly become too low and dizzy or faint.

3) The whole process is about 45-60 minutes:
It's not that they are slow, the medical staff are very professional, and the whole process is very smooth. It takes so long because we will go through at least 5 different stations:
-Confirm the name and scan mysejahtera
-The medical staff will explain to you the possible side effects and sign the consent form
-vaccination
-Observe for 30 minutes
-Schedule the second dose
-Self-timer before leaving (usually the longest time)

✔️After vaccination:

1) Adverse reactions/sequelae:
It can be solved by itself: such as headache, dizziness, fever, general weakness, muscle aches, redness and pain at the injection site, if necessary, you can take paracetamol=panadol (cannot take NSAIDs, which will reduce the effect of the vaccine), and take more rest.

2) When do I need to go to the hospital?
Regardless of the vaccine, if you have difficulty breathing, swollen eyes, swollen face, fast heartbeat, and severe wind foam all over your body, please seek medical attention as soon as possible.

For people vaccinated with Astrazeneca, within 4 days to 4 weeks after receiving the vaccine, if the following symptoms occur, please seek medical treatment immediately.
1. Painkillers do not relieve severe headaches
2. Headache worsens when lying down or bending over
3. Unusual headache with blurred vision, nausea and vomiting
4. There are small spots on the skin, and black and blue will appear if it is not hit
5. Difficulty breathing, chest pain, swelling of the legs, or persistent abdominal pain

3) Things that are not recommended after vaccination (basically there are not too many restrictions):
-It is ridiculous to not be able to take a bath for 24 hours after vaccination.
-In terms of diet, it may be recommended to avoid irritating foods within 24 hours, mainly light.
(Recently, after a friend was vaccinated, he immediately went to eat seafood spicy hot pot. As a result, he developed severe allergies and needed to be treated in the hospital. The problem here is that we don’t know if this allergy is caused by seafood or a vaccine. , And this will also affect whether you can get the second dose of vaccine in the future.)

Finally, I still want to remind everyone that after vaccination, it usually takes 1-2 weeks for the immune system to produce antibodies. Please don't take it lightly. Even after two doses, there is still a chance of being infected, but the symptoms will not be so severe, so it is very important to take a good protection sop.
