KLANG - [DIKEMASKINI] Seorang anggota polis dari Ibu Pejabat Polis Daerah (IPD) Klang Selatan yang sedang menjalani kuarantin di rumah di sini meninggal dunia pada Rabu akibat jangkitan Covid-19.

Ketua Polis Daerah Klang Selatan, Asisten Komisoner Shamsul Amar Ramli berkata, pihaknya dimaklumkan kejadian itu selepas menerima panggilan daripada MERS 999 pada jam 8.29 malam.

"Arwah sebelum ini telah disahkan positif selepas menjalani ujian saringan Covid-19 pertama pada 22 Januari kerana mengalami demam.

"Ujian saringan kedua pada keesokan harinya turut disahkan positif," katanya kepada Sinar Harian pada Khamis.

Shamsul Amar berkata, Allahyarham berusia 53 tahun telah menjalani kuarantin di rumahnya di Klang bermula pada 22 Januari hingga 1 Februari, namun beliau disahkan meninggal dunia pada Rabu.

Katanya, sebab kematian yang disahkan oleh Pegawai Perubatan Hospital Tengku Ampuan Rahimah (HTAR) Klang, Dr Tian Pei Xuan adalah kerana Covid-19.

Perintah kawalan pergerakan bersyarat (PKPB) untuk Sabah dan Lembah Klang sebelum ini masing-masing ditetapkan akan berakhir pada 26 dan 27 Okt.

Bagaimanapun, Menteri Kanan (Kluster Keselamatan) Ismail Sabri Yaakob berkata situasi masih boleh berubah-ubah.

Kerajaan masih belum tahu sama ada PKPB akan disambung atau berakhir pada tarikh yang dinyatakan itu.

MIRI: Sarawak's Covid-19 task force has found that 2,484 people in Kuching, believed to be close contacts of positive Covid-19 patients, have not undergone any health screenings.

State Disaster Relief Management Committee said the task force will be contacting the 2,484 to undergo screening starting Monday (April 20).

The committee said the task force will get the help of the State Health Department to call each of the 2,484 close contacts.

"The task force is giving number one priority to tracing and getting those people to undergo immediate tests to determine if they are infected or not.

"This is a top priority in efforts to reduce Covid-19 infections.

"Until today, 3,538 close contacts of Covid-19 positive patients had been sent for tests in Kuching.

"Kuching has the highest number of infections in the state," said the committee.

The committee said the 2,484 people must obey instructions to undergo tests without delay.

If they refuse to voluntarily submit themselves, stricter measures will be taken to compel them to undergo mandatory tests, it warned.

Former state health directors Dr Jamilah Hashim and Datuk Dr Andrew Kiyu were recruited last week by state authorities to head the task force to try and reduce the Covid-19 surge in Sarawak that has claimed 14 lives so far, while more than 400 other confirmed positive cases are being treated in hospitals statewide.

Asked on the possibility of employers forcing their workers to return to work during the MCO, Ismail Sabri who is also Defence Minister said legal action could be taken against the employers.

Commenting on offshore workers returning home during the enforcement of the MCO, he said they must undergo the 14-day mandatory quarantine at designated quarantine centres.

“Our policy is whoever comes in, including those returning home, will be placed under quarantine,” he said.

PETALING JAYA: Today marks the end of the first four weeks that Malaysia has been under the movement control order (MCO).

Although close to a month has gone by since the introduction of rules to curb the spread of Covid-19, health experts are divided on whether the country has seen the peak of the disease, which had resulted in 4,817 confirmed cases as at noon yesterday.

Universiti Sains Malaysia medical epidemiologist and biostatistician Assoc Prof Dr Kamarul Imran Musa believes that the actual number of Covid-19 cases in Malaysia is higher right now – as much as four to 10 times more than the number of confirmed cases – because there are asymptomatic cases (people infected with the virus but do not have symptoms of the disease) that are not being tested.

“The peak of infection reflects the highest rate of the spread of the virus, but it is very unlikely we would see a classic pointed peak of Covid-19 spread in Malaysia.

“This is because the number of new confirmed cases hover between 110 and 200 daily and the spread of the infection in the community has stabilised, ” he said.

Dr Kamarul added that Malaysia would not see a pointed peak as seen in South Korea as the Koreans had tested aggressively from the beginning, thus leading to a rapid decrease in its infection rates.

“I expect in Malaysia’s setting, we would stay in this ‘peak’ for a much longer time, ” he said.

In a March 23 research paper, investment bank JP Morgan forecasted that Malaysia’s peak infections would happen in mid-April with about 6,300 cases.

Several experts back then said the forecast was conservative.

Dr Kamarul noted that Covid-19 cases would fall if the country could break the transmission of the virus.

“The constant number of new cases between 100 and 200 per day indicates that the infection is still spreading within the community but the good thing is that the rate of infection has stabilised, ” he said.

He said the MCO had to a certain extent halted the speed of infections in the country.

Without it, Dr Kamarul said, Covid-19 would have spread at a much greater rate.

Universiti Malaya virologist Prof Dr Sazaly Abu Bakar said it would take another two weeks to know whether Covid-19 had peaked or not.

“Now that we have loosened the restrictions by allowing certain sectors of the economy to start working again, I don’t know what’s the impact. The next two weeks will be critical for the country. If we don’t see a downward trend, that means we have not been able to push the infections down, ” he said.

Like Dr Kamarul, Dr Sazaly believed that the actual number of positive cases in the country was unknown because of the lack of aggressive testings.

“All those numbers that have been reported every evening could be misleading as they relate to symptomatic patients who require treatment in the hospital.

“As for those who are positive but are asymptomatic, we don’t know how many of them are out there because we don’t do aggressive testing.

“Due to that, we don’t know where the trend is heading, ” he said.

He added that the number of Covid-19 cases in the country was currently “fluctuating upwards rather than downwards”.

“If it fluctuates around 80 to 90 cases per day, then it is okay. It means that it is fluctuating down. But in the last two week of the MCO, it has still been fluctuating at the higher end. The trend is still upwards, ” he said.

Universiti Putra Malaysia medical epidemiologist and biostatistician Assoc Prof Dr Malina Osman said she could not be sure whether the disease had peaked in Malaysia.

In fact, she cannot rule out the possibility of a third wave of new cases.

Dr Malina said the new cases could come from sub-clusters from identified major clusters, those returning from overseas, non-citizens (such as refugees), neighbouring countries, and even frontliners.

However, she believed that active cases and new cases could be seen plateauing in the country, adding that active preventive actions would prevent a third wave.

Autopsies show severe damage to COVID-19 patients' lungs and immune system, according to a doctor in Wuhan reached by the Global Times, who called for measures to prevent fibrosis of the lungs at an early stage of the disease.

"The influence of COVID-19 on the human body is like a combination of SARS and AIDS as it damages both the lungs and immune systems," Peng Zhiyong, director of the intensive care unit of the Zhongnan Hospital of Wuhan University in Wuhan, told the Global Times on Friday.

Peng said he had just talked to Liu Liang, a forensic specialist from the Tongji Medical College at Huazhong University of Science and Technology. Liu's team has reportedly conducted nine autopsies on deceased COVID-19 patients as of February 24.

"The autopsy results Liu shared inspired me a lot. Based on the results, I think the most important thing now is to take measures at an early stage of the disease to protect patients' lungs from irreversible fibrosis," Peng noted.

If irreversible damage is done, other measures, like those to prevent patients from oxygen deficit, will not be of much use, he said.

Liu's team published a paper on an autopsy they conducted in the Journal of Forensic Medicine on Tuesday.

The paper said there was apparent damage to the patient's lungs. An excess production of mucus spilled out of the alveoli, indicating COVID-19 causes an inflammation response that damages deep airways and pulmonary alveoli.

The patient, an 85-year-old man, exhibited similar pathological changes to those caused by SARS and MERS. Fibrosis in his lungs was not as serious as was seen in SARS patients, but an exudative reaction was more apparent, possibly due to the short course of his disease.

News about the paper went viral on Chinese social media platforms on Friday.

Some news reports said that Liu's team's autopsies showed that sputum bolt is one the main reasons that caused COVID-19 patients' deaths. Some medical staff have used sputum aspirators for patients based on the results, which led to a decline by half in the number of deaths in Wuhan on Wednesday.

The National Health Commission (NHC) said Wuhan reported 42 deaths on Tuesday and 19 on Wednesday.

However, Peng doubted the reports, saying that he believes the decline was due to the improving situation and shrinking patients pool.

"Even if the autopsy results are helpful, the effectiveness would not come out so quickly. It takes at least one week to observe," Peng said.

He warned that the number of deaths may rebound as he knows many critical patients in Wuhan have held out for one month but are still in danger and could die anytime.

There were 6,775 critical patients in Wuhan as of Thursday and the number of deaths Wuhan authority reported on Thursday was 28, according to NHC.

The patient mentioned in the paper was hospitalized in January for a cerebral infraction, the death of tissue in the brain resulting from inadequate blood supply. He was confirmed to be infected with novel coronavirus 13 days later and died after a further 15 days. The autopsy was conducted within 12 hours of his death.

No apparent pathological changes were found in his digestive system, spleen or brain, the report said.

Liu's team conducted the first on February 16, about a month after the outbreak.

Liu previously told media that he and his team had proposed to the Wuhan government to conduct autopsies soon after the outbreak.

The government and local hospitals agreed on the necessity of autopsies but could not provide proper locations. They also worried about risks of viral transmission during such autopsies.

Yang Zhanqiu, deputy director of the pathogen biology department at Wuhan University, explained that preparation and risk evaluation of the autopsy of COVID-19 patients is crucial, as it may cause containment of the hospital or medical members.

Also, Chinese traditional thoughts of preserving the body of the deceased may hinder the autopsy process, which requires family consent, Yang told the Global Times.

He was commenting on the statements made by Barisan Nasional, PAS and PBRS leaders after their meeting with the Yang di-Pertuan Agong on Tuesday (Feb 25).

Umno secretary-general Tan Sri Annuar Musa had said the parties had initially agreed to form a coalition government headed by Dr Mahathir – on condition that DAP would not be part of it.

He said that they had retracted their statutory declaration in support for Dr Mahathir, adding that they also rejected the latter's proposal for a unity government.

Annuar, who is Ketereh MP, said Umno, PAS, MCA, MIC and PBRS had unanimously agreed that a dissolution of Parliament was the only way forward.

Former prime minister Datuk Seri Najib Razak and Umno president Datuk Seri Dr Ahmad Zahid Hamidi are facing a slew of corruption charges in relation to their tenure in the previous government.

Mukhriz further said that as much as they wanted to join Dr Mahathir to form a government, Barisan Nasional leaders have yet to state that they will not be supporting corrupt leaders.

"To date, we have not heard from Umno and Barisan that if they were to form a government (with Bersatu), the trials involving their former president Najib, his wife Datin Seri Rosmah Mansor and present president Ahmad Zahid will continue without interference, and that any verdict will be accepted by them," said Mukhriz.

In the past few weeks, MPs from both sides of the then Pakatan Harapan ruling coalition and opposition parties including Barisan signed SDs stating their support for Dr Mahathir to complete a full term as prime minister.

Political developments then saw Parti Pribumi Bersatu Malaysia leaving Pakatan, defections from PKR and the coming together of MPs from both sides of the divide in support of Dr Mahathir as the premier.

On Tuesday (Feb 25), Dr Mahathir tendered in his resignation as Prime Minister and was accepted by the Yang di-Pertuan Agong.

However, the King has consented for Dr Mahathir to continue running the country as interim Prime Minister until a new premier has been appointed and a new Cabinet formed.

The King is currently in the midst of interviewing all MPs to determine who commands the majority of support in Parliament.
