How was the interview process?

Resignation problems

Hi, I have questions about resignation:

1) In the letter, for the ATTENTION: part I have to write the relevant manager name or just the title?such as Shift Manager. Can I include 2 persons or more in the ATTENTION: such as ATTENTION: Shift Manager / Operations Manager / HR Manager like this?

2) When I write the effective date and notice period, do I have to write the end date of notice period as below?

Please be informed that effective 30 SEPTEMBER 2013, I am resigning from your company as [position title] provided for in my employment contract. Henceforth, I will serve the designated 2 month notice period as per required by my contract until the expiration of my notice which is 30 NOVEMBER 2013.

Coz I plan to submit the resignation letter on 30 September 2013, so is it the way I wrote is correct?

Recently I have found Hilti Asia IT Services Sdn. Bhd. job posting in jobstreet, I saw the above requirement, is it really a must for CGPS>3.5 or similar grading? May I know anyone who do not have CGPS>3.5 or similar grading tried to apply and successfully to interview and finally hire by them?
