The plastic tubes delivering oxygen through his nose hardly seemed adequate to stop his chest from heaving. He looked exhausted.&lt;br /&gt;&lt;br /&gt;He had tested positive for the coronavirus 10 days ago. He was under 50, mildly hypertensive but otherwise in good health. Eight days earlier he started coughing and having severe fatigue. His doctor started him on antibiotics. It did not work.&lt;br /&gt;&lt;br /&gt;Fearing his symptoms were worsening, he started taking some hydroxychloroquine he had found on the internet. It did not work.&lt;br /&gt;&lt;br /&gt;He was now experiencing shortness of breath while doing routine daily activities such as walking from his bedroom to the bathroom or putting on his shoes. He was a shell of his former self. He eventually made his way to a facility where he could receive monoclonal antibodies, a lab-produced transfusion that substitutes for the body’s own antibodies. It did not work.&lt;br /&gt;&lt;br /&gt;He finally ended up in the ER with dangerously low oxygen levels, exceedingly high inflammatory markers and patchy areas of infection all over his lungs. Nothing had helped. He was getting worse. He could not breathe. His wife and two young children were at home, all infected with the virus. He and his wife had decided not to get vaccinated.&lt;br /&gt;&lt;br /&gt;Last year, a case like this would have flattened me. I would have wrestled with the sadness and how unfair life was. Battled with the angst of how unlucky he was. This year, I struggled to find sympathy. It was August 2021, not 2020. The vaccine had been widely available for months in the U.S., free to anyone who wanted it, even offered in drugstores and supermarkets. Cutting-edge, revolutionary, mind-blowing, lifesaving vaccines were available where people shopped for groceries, and they still didn’t want them.&lt;br /&gt;&lt;br /&gt;Outside his hospital door, I took a deep breath — battening down my anger and frustration — and went in. I had been working the COVID-19 units for 17 months straight, all day, every day. I had cared for hundreds of COVID patients. We all had, without being able to take breaks long enough to help us recover from this unending ordeal. Compassion fatigue was setting in. For those of us who hadn’t left after the hardest year of our professional lives, even hope was now in short supply.&lt;br /&gt;&lt;br /&gt;Shouting through my N95 mask and the noise of the HEPA filter, I introduced myself. I calmly asked him why he decided not to get vaccinated.&lt;br /&gt;&lt;br /&gt;“Well, I’m not an anti-vaxxer or anything. I was just waiting for the FDA to approve the vaccine first. I didn’t want to take anything experimental. I didn’t want to be the government’s guinea pig, and I don’t trust that it’s safe,” he said.&lt;br /&gt;&lt;br /&gt;“Well,” I said, “I can pretty much guarantee we would have never met had you gotten vaccinated, because you would have never been hospitalized. All of our COVID units are full and every single patient in them is unvaccinated. Numbers don’t lie. The vaccines work.”&lt;br /&gt;&lt;br /&gt;This was a common excuse people gave for not getting vaccinated, fearing the vaccine because the Food and Drug Administration had granted it only emergency use authorization so far, not permanent approval. Yet the treatments he had turned to — antibiotics, monoclonal antibodies and hydroxychloroquine — were considered experimental, with mixed evidence to support their use.&lt;br /&gt;&lt;br /&gt;The only proven lifesaver we’ve had in this pandemic is a vaccine that many people don’t want. A vaccine we give away to other countries because supply overwhelms demand in the U.S. A vaccine people in other countries stand in line for hours to receive, if they can get it at all.&lt;br /&gt;&lt;br /&gt;“Well,” I said, “I am going to treat you with remdesivir, which only recently received FDA approval.” I explained that it had been under an EUA for most of last year and had not been studied or administered as widely as COVID-19 vaccines. That more than 353 million doses of COVID-19 vaccine had been administered in the U.S. along with more than 4.7 billion doses worldwide without any overwhelming, catastrophic side effects. “Not nearly as many doses of remdesivir have been given or studied in people and its long-term side effects are still unknown,” I said. “Do you still want me to give it to you?”&lt;br /&gt;&lt;br /&gt;“Yes” he responded, “Whatever it takes to save my life.”&lt;br /&gt;&lt;br /&gt;It did not work.&lt;br /&gt;&lt;br /&gt;My patient died nine days later of a stroke. We, the care team, reconciled this loss by telling ourselves: He made a personal choice not to get vaccinated, not to protect himself or his family. We did everything we could with what we had to save him. This year, this tragedy, this unnecessary, entirely preventable loss, was on him.&lt;br /&gt;&lt;br /&gt;The burden of this pandemic now rests on the shoulders of the unvaccinated. On those who are eligible to get vaccinated but choose not to, a decision they defend by declaring, “Vaccination is a deeply personal choice.” But perhaps never in history has anyone’s personal choice affected the world as a whole as it does right now. When hundreds and thousands of people continue to die — when the most vulnerable members of society, our children, cannot be vaccinated — the luxury of choice ceases to exist.&lt;br /&gt;&lt;br /&gt;If you believe the pandemic is almost over and I can ride it out, without getting vaccinated, you could not be more wrong. This virus will find you.&lt;br /&gt;&lt;br /&gt;If you believe I’ll just wait until the FDA approves the vaccine first, you may not live to see the day.&lt;br /&gt;&lt;br /&gt;If you believe if I get infected I’ll just go to the hospital and get treated, there is no guarantee we can save your life, nor even a promise we’ll have a bed for you.&lt;br /&gt;&lt;br /&gt;If you believe I’m pregnant and I don’t want the vaccine to affect me, my baby or my future fertility, it matters little if you’re not alive to see your newborn.&lt;br /&gt;&lt;br /&gt;If you believe I won’t get my children vaccinated because I don’t know what the long-term effects will be, it matters little if they don’t live long enough for you to find out.&lt;br /&gt;&lt;br /&gt;If you believe I’ll just let everyone else get vaccinated around me so I don’t have to, there are 93 million eligible, unvaccinated people in the “herd” who think the same way you do and are getting in the way of ending this pandemic.&lt;br /&gt;&lt;br /&gt;If you believe vaccinated people are getting infected anyway, so what’s the point?, the vaccine was built to prevent hospitalizations and deaths from severe illness. Instead of fatal pneumonia, those with breakthrough infections have a short, bad cold, so the vaccine has already proved itself. The vaccinated are not dying of COVID-19.&lt;br /&gt;&lt;br /&gt;SARS-CoV-2, the virus that causes COVID-19, has mutated countless times during this pandemic, adapting to survive. Stacked up against a human race that has resisted change every step of the way — including wearing masks, social distancing, quarantining and now refusing lifesaving vaccines — it is easy to see who will win this war if human behavior fails to change quickly.&lt;br /&gt;&lt;br /&gt;The most effective thing you can do to protect yourself, your loved ones and the world is to GET VACCINATED.&lt;br /&gt;&lt;br /&gt;And it will work.&lt;br /&gt;&lt;!--QuoteEnd--&gt;&lt;/div&gt;&lt;!--QuoteEEnd--&gt;&lt;!--SPOILER DIV--&gt;&lt;/div&gt;&lt;!--SPOILER DIV--&gt;</description>

Patient 91, a British pilot, almost died.&lt;br /&gt;&lt;br /&gt;Now there is a new number that has become an unwelcome marker.&lt;br /&gt;&lt;br /&gt;Patient 416, a 57-year-old man in the central city of Danang, tested positive for coronavirus last week after more than three months without a new infection in the Southeast Asian country.&lt;br /&gt;&lt;br /&gt;In just over a few days, a total of 47 people linked to Danang have tested positive, signalling the start of a third, untraceable and more concerning wave of infections in a country that had reopened faster than most of the world, confident it had beaten the disease.&lt;br /&gt;&lt;br /&gt;The surge has spread to six cities and provinces in six days, including Hanoi, Ho Chi Minh City and the Central Highlands.&lt;br /&gt;&lt;br /&gt;Vietnam&amp;#39;s Prime Minister, Nguyen Xuan Phuc, slammed his fist on the table during a meeting on Wednesday in which he warned that the entire country was now at risk of infection.&lt;br /&gt;&lt;br /&gt;&amp;quot;Everyone, including health workers, had let their guard down months before the reintroduction of the virus,&amp;quot; said Truong Huu Khanh, head of the infectious diseases department at the Nhi Dong I Hospital in Ho Chi Minh City.&lt;br /&gt;&lt;br /&gt;&amp;quot;People had stopped wearing masks in public and some health workers didn&amp;#39;t pay enough attention to &amp;#39;high risk&amp;#39; cases&amp;quot;.&lt;br /&gt;&lt;br /&gt;So successful was its fight against COVID-19 that the post-pandemic &amp;quot;new normal&amp;quot; in Vietnam looked very much like the old one. Bars, restaurants and hotels had reopened with minimal social distancing measures in place. In early July, the number of domestic air passengers even rose by 27% compared to the previous year, according to state media.&lt;br /&gt;&lt;br /&gt;But that oasis in a pandemic-stricken world began to disintegrate when Patient 416 went to a local hospital in Danang with a cough and fever. The last person to test positive for an infection in the community was a 16-year-old girl in April.&lt;br /&gt;&lt;br /&gt;Patient 416 was initially diagnosed with pneumonia, according to Vietnam&amp;#39;s health ministry, and did not test positive for the coronavirus until July 23. Days earlier, he had visited other hospitals in Danang to help his ailing mother, and attended one party and a wedding.&lt;br /&gt;&lt;br /&gt;Vietnam&amp;#39;s outbreak had been under control, thanks to an aggressive contact-tracing system and a centralised quarantine programme, which had kept the coronavirus tally to under 500 cases, with no deaths.&lt;br /&gt;&lt;br /&gt;&amp;quot;We acted fast and fiercely back in March but the virus is still breaking out all over the world,&amp;quot; said Khanh.&lt;br /&gt;&lt;br /&gt;&amp;quot;We can&amp;#39;t let our guard down. This third wave is the cost for a moment of complacency&amp;quot;.&lt;br /&gt;&lt;br /&gt;TESTING PATIENCE&lt;br /&gt;&lt;br /&gt;The origin of the outbreak in Danang is unclear.&lt;br /&gt;&lt;br /&gt;News of Patient 416&amp;#39;s infection prompted the mass exodus of over 80,000 tourists from the popular resort city. On Thursday, authorities in Hanoi said they would begin the mass testing of 20,000 of those tourists who had returned to the capital, as Vietnam&amp;#39;s human-powered contact tracing system was redeployed.&lt;br /&gt;&lt;br /&gt;Some of the latest cases, including Patient 449, an American citizen based in Danang who appears to have developed symptoms in mid-June, indicate the virus had been quietly spreading in the city for several weeks.&lt;br /&gt;&lt;br /&gt;It was not clear when Patient 449, who the health ministry said suffered from chronic pneumonia, was tested for the virus, but he was treated for COVID-like symptoms at three different hospitals before testing positive at a fourth hospital in Ho Chi Minh City.&lt;br /&gt;&lt;br /&gt;The health ministry stopped publishing testing data in mid-May, when 260,000 tests had been carried out.&lt;br /&gt;&lt;br /&gt;On Monday, the ministry said it had completed 430,000 tests since the pandemic began, indicating that testing continued to rise as thousands of returning overseas Vietnamese were quarantined.&lt;br /&gt;&lt;br /&gt;It was not clear, however, to what extent COVID-19 testing outside quarantine had continued. Khanh said some outside testing, especially on patients with pneumonia symptoms, had been in effect but on a significantly smaller scale.&lt;br /&gt;&lt;br /&gt;Although the two issues have not been officially linked, the government has over recent days made several strongly-worded statements about illegal immigration, indicating that the source of the new infections may have come from overseas.&lt;br /&gt;&lt;br /&gt;&amp;quot;I think it&amp;#39;s most likely a reintroduction of the virus from elsewhere, sometime in the last two months, but how, when, or where remain entirely speculative at present,&amp;quot; said Guy Thwaites, director of the Oxford University Clinical Research Unit in Ho Chi Minh City.&lt;br /&gt;&lt;br /&gt;&amp;quot;What is happening now in Vietnam was almost inevitable.             <title>M&amp;#39;sia should repeal Health Circular 10/2001</title>

            <description>&lt;a href='http://english.astroawani.com/malaysia-news/covid-19-demonstrates-harm-exclusion-healthcare-msia-should-repeal-health-circular-10-2001-235149' target='_blank'&gt;http://english.astroawani.com/malaysia-new...-10-2001-235149&lt;/a&gt;&lt;br /&gt;&lt;br /&gt;IF the government wants to create more trust among refugee and migrant communities to come forward and get screened for COVID-19, they should officially repeal Circular 10/2001, under which healthcare providers are obliged to report “illegal immigrants” (pendatang asing tanpa izin, or PATI) seeking healthcare services to the police and immigration authorities.&lt;br /&gt;&lt;br /&gt;In recent weeks, the government introduced additional measures to contain the COVID-19 outbreak in Malaysia. Of particular concern were those who joined the tabligh gathering at Sri Petaling Mosque in February. The meeting was attended by thousands, including several hundred refugees and migrants, some without legal documents. The government has urged everyone to be screened. Even if many amongst the refugee and migrant communities have come forward, they faced serious barriers.&lt;br /&gt;The government’s decision to ensure free access to COVID-19 testing for all is a move in the right direction. Asylum seekers are normally required to pay the much higher “foreigner rates” for healthcare. A 50% discount for documented refugees is still based on these foreigner rates. Refugees and asylum-seekers however also live with a constant fear of being arrested and detained. This fear exists at home and in the street – as immigration raids are frequent- but even at health care facilities.&lt;br /&gt;&lt;br /&gt;This fear is not unfounded. Undocumented migrants have been living under a constant threat of arrest when visiting healthcare facilities since Circular 10/2001 was first put in place almost 20 years ago. As a medical humanitarian organisation providing healthcare to refugees and vulnerable populations, Médecins Sans Frontières (MSF) has witnessed how the fear of being reported to Immigration services has deterred many patients from seeking proper, timely treatment. In the primary healthcare clinic in Penang supported by MSF, patients from the Rohingya and other refugee communities also report leaving hospitals or absconding during treatment after being threatened of arrest.&lt;br /&gt;An analysis conducted by MSF in Penang between May and December 2017 found that more than 20% of the people MSF referred to UNHCR for assessment of their asylum claim, had been refused admission or registration at a healthcare facility in whole or in part on the basis of their immigration status. Several patients reportedly were arrested from health care facilities and detained. One patient was arrested and detained for two months, when she went to register her pregnancy at a government clinic in her final trimester. MSF continues to encounter patients with life-threatening conditions who report delays seeking treatment due to a fear of being reported to Immigration services. In 2018 this included 3 people with suspected TB.&lt;br /&gt;&lt;br /&gt;It’s commendable that the announcement by Defense and Senior Minister Dato&amp;#39; Sri Ismail Sabri on 22 March confirmed a government agreement neither to look into legal status nor take any action for being in the country without legal status during COVID-19 screening. However, the fact that these statements are so strongly linked to the tabligh gathering make it unclear for those individuals not linked to the event.&lt;br /&gt;&lt;br /&gt;MSF therefore calls for an unequivocal repeal of Circular 10/2001 by the Ministry of Health. This is also the right time for other concerned ministries and government agencies, such as the Home Ministry, National Security Council and Immigration Department, to express the political will to support this initiative. The challenge Malaysia now faces in achieving better contact tracing during the COVID-19 outbreak clearly illustrates the negative impact of no separation between access to medical care and Immigration services. Repealing Circular 10/ 2001 will allow health care providers to practice their professional and ethical obligation: provide medical care to patients in need, regardless of immigration status.&lt;br /&gt;&lt;br /&gt;A repeal of Circular 10/ 2001 would send a clear message that everyone should have access to healthcare. Such a move could help to prevent similar delays in healthcare-seeking in the future, ensuring effective public health for the entire population. Malaysia has a robust health care system. That a 20-year-old policy limits access to essential care and negatively impacts on public health measures by marginalising vulnerable groups is harmful and must change.&lt;br /&gt;&lt;br /&gt;COVID-19 shows us that policies of social exclusion and limited access to health care can impact society as a whole.             <description>&lt;a href='https://www.thestar.com.my/business/business-news/2018/06/25/glc-singers-sing-a-different-tune/' target='_blank'&gt;https://www.thestar.com.my/business/busines...different-tune/&lt;/a&gt;&lt;br /&gt;&lt;br /&gt;PETALING JAYA: The heads of government linked companies (GLC) who sang a song that later became the theme song for the Barisan Nasional’s election campaign have distanced themselves from the controversial music video.&lt;br /&gt;&lt;br /&gt;Those who sang and played musical instruments in the music video titled “Hebat Negaraku” (my country is great) said they did not know the video or the song was going to be a political theme song.&lt;br /&gt;&lt;br /&gt;There have been repercussions on the CEOs who appeared in the music video. They have come under scrutiny for making a song that was used as propaganda by Barisan in the last general election.&lt;br /&gt;&lt;br /&gt;Three of the GLC bosses in the video have either retired or resigned since the new government took over.&lt;br /&gt;&lt;br /&gt;Several more have been speculated to leave in the coming weeks or months but nothing is cast in stone. Sources said this is because most of the CEOs are not known to have campaigned openly for either Barisan or Pakatan Harapan.&lt;br /&gt;&lt;br /&gt;“None of the CEOs had a clue it would become a political song. Do you really think the CEOs would have done it if they knew it would become political?” asked one of the CEOs who appeared in the video but declined to be named.&lt;br /&gt;&lt;br /&gt;“We have said no to so many things, and we could have easily have said no to this if it was political.’’&lt;br /&gt;&lt;br /&gt;Another CEO said he was approached and felt it was “more of a patriotic song and nothing more.”&lt;br /&gt;&lt;br /&gt;“At that point in time, we did not think much (of the repercussions). Hebat Negaraku was announced as Barisan’s campaign theme long after the recording was made. We did not know that.’’&lt;br /&gt;&lt;br /&gt;Another CEO added: “We thought it was a casual thing when we were approached as some of the CEOs have their own band.’’&lt;br /&gt;&lt;br /&gt;It all started when several CEOs were called to be part of a music video and they thought it was to showcase the musical talents of 14 GLCs heads, plus staff members of the 20 key GLCs.&lt;br /&gt;&lt;br /&gt;The song is about the greatness, advancement and inspiration of Malaysia. It was released on YouTube on March 22 but has since been taken down.&lt;br /&gt;&lt;br /&gt;But fingers have been pointed at the GLCs bosses who made the music video because it became a political video.&lt;br /&gt;&lt;br /&gt;Datuk Seri Shazalli Ramly has been said to be the main orchestrator for the group in terms of making the music video. He was also said to be the branding chief for Barisan’s elections campaign.&lt;br /&gt;&lt;br /&gt;Barisan lost the elections held on May 9 to Pakatan, which has since formed a new government and is scrutinising all the performance, processes, remuneration and procurement of the government and GLCs.&lt;br /&gt;&lt;br /&gt;Shazalli quit his job as group CEO of Telekom Malaysia Bhd &amp;#153; on June 6. Malaysian Resources Corp Bhd (MRCB) group managing director Tan Sri Mohamad Salim Fateh Din has retired as group MD last week and it was something he had planned to do. Malaysia Airports Holdings Bhd Datuk Badlisham Ghazali did not get his contract renewed. All three were in the music video.&lt;br /&gt;&lt;br /&gt;There is a GLC secretariat that now comes under the purview of TM, which was earlier parked under Khazanah Nasional Bhd. The secretariat organised the making of the music video, according to sources. The CEOs were called to attend a session and within a few hours it was all done with no prior rehearsals.&lt;br /&gt;&lt;br /&gt;“When you are called, it could be difficult not to comply since it is the secretariat that called you. We have to oblige but we really did not know it was going to be a campaign slogan. This is really unfortunate that it has turned out like this.&lt;br /&gt;&lt;br /&gt;“We were surprised when we found out it was a party slogan but it had already been done and what can we do, we are in the picture,’’ said another CEO.&lt;br /&gt;&lt;br /&gt;Not all CEOs who were invited took part in the video. Prior engagements were the reason used for declining to appear.</description>
