Form select button to insert info to db mysql

I need help here.. I want to create some form for submit data to db. I have no problem with the form and inserting data to db. but I stuck when I want to add new value to db table collum. I have create 1 select button and on the select button value list from db collum (e.g category). picture attached.

Top: select the value from db and input disable
bottom: select create new and input enable to create new value

how to do this.. Hope u guys clear and can help me.. thanx in advance!

sorry for complicate explaination..

i want to create one select button for choosing category.. select from existing category or create new one.. for create new one.. select "create new" from select button and enable text input for naming the new category.. for listing the existing category im all done but im stuck on creating new category..

the picture i attach is the same select button but on different situation..

Pc cant start.. power button blinking

help..

my pc suddenly lost power this morning.. it turn off while im wroking and not start again..
i try push the power button normally for turn on the pc.. and the power is there but

the power button light is blinking
blue and red light also blinking
i push/hold the power button to turn off power is not working
fans & cpu fan is running..
power led on mobo is on..

any idea what the problem is? please help.. thanks...
