Income Tax Advise

Hi all, looking for some advice regarding Income Tax in Malaysia.

Here is my scenario:-

1) I was employed from December 2012 to June 2016.

2) I was promoted on January 2016 and resigned in June 2016.

3) Now im currently unemployed from June 2016 to present.

4) Only ever provided with ONE EA Form from company HR once which was for the year ended 31 December 2015 and Total amount read RM59K. I took the form and did nothing with it (Lol).

5) I believe before that not provided any EA Form by company HR as salary too low, not sure.

6) I have also received a letter in 2013 from LHDN which was sent via post to my address which states my "nombor cukai pendapatan". Again, took the form a did nothing with it (Lol).

7) I do not have a PIN to do any online tax submissions as I never attended to LHDN to obtain PIN.

From the above, what should I do now?

I know I should head to LHDN to firstly obtain my PIN, and then what should I do?

Do i need to file taxes for the period December 2012 to June 2016? Any fines to be incurred and roughly how much in this instance?

Now that I am unemployed am I entitled to any refunds and what not?

I'm lost and I know nothing about the income tax procedures here. Appreciate any help.
