Career Break - should I inform potential employer?

Hi, I need your opinion on this subject.

In my resume, I have more than 10 years of experience in working.

For my previous job however, I only lasted for 3 months due to work incompatible with Head Department, unnecessary stress (due to shouting and belittling remarks), high turnover among own colleagues (which later becomes whoever stays' fault). I worked till January only.

I spent February & March to heal myself. I have never been anxiety for my jobs, panics, sleep (hearing voices), and have so much fear for having a job.
It was a nightmare. I'm glad I ended my period.

So I'm ready to go back to employment.

During pre-screening and interview session, I'm quite hesitate to explain why I last short period..
Normally I just declare I'm still working.

I have a few questions and need your view on how should I answer these questions.

Q1: Why do you seek new employment?
Q2: Are you currently still employed?
Q3: What is your expected salary?
Q4: When is your availability?
Q5: Can we check with your reference?

I normally answer like this:
A1: I tell the truth on what has happened in office. It makes me think there is no job stability.
A2: Normally, I say Yes still under employed. I don't want to be seen as irresponsible or portray the bad side of me like I run away....
A3: 10% of increment from previous salary since I work 3 months (previous job)
A4: 14 days due to I haven't been confirmed..... let's say if I say I'm on career break, means I can start immediately?
A5: I will give reference from previous employer which I trusted more.... I couldn't have share with the not reliable sources, right?

The questions are simple, but one lead to another like a chain answers...
I hope I stand better chance if I could answer in better words.
